If someone has the habit of drinking and smoking regularly, what are the best health insurance policies (comprehensive or regular) out there, that can protect him/her against any liver or other relevant organ diseases arising out to these habits?

There will be a element of loading charges that insurer will impose on you if you're a smoker or consume alcohol.

If the insurer thinks you're habituated and there is presendence of hospitalization linked to consumption of alcohol/cigarettes then the insurer may also reject your application.

I am confused here because all policies I saw has permanent exclusion saying diseases or hospitalization because of substance abuse(smoking or drinking) will not be covered. For ex, Ergo Optima Secure

Given that,

1. What is the point of declaration and subsequent premium loading?

2. Given they approved the policy after declaration, will they cover non-substance related claims only or even substance related claims?
