Details of medical history of person assured and family members in the MEDICAL FORM

After using beshak platform, I was able to select a term plan for me. Thanks for this great platform and services.
My question is, I declared every known family and my medical history in my application form and then sample picking for medical exam from my home was arranged. A phlebotomist came over for the sample collection. After taking the samples, he asked some basic details like my height, weight, corona infection, chest-waist measurement, basic medical condition of me like whether I had BP, diabetes, Asthma etc and wrote it down in a medical form. He asked for my signatures under different sections of the form which were either not filled or not fully/accurately filled i.e. there was a section in which medical history of my family was asked and he did not fill it. When I asked him about it, he told me that after your medical results the doctor will fill the details in it. I did not understand it.
So, is it my liability or fault if the doctor does not accurately fill all the details in the MEDICAL form even though I had already given every information in my Application/Proposal form.

The basis of insurance contract is Proposal form which need to be filled or verified by proposer I.e ypu .
It is your responsibility hence read all contents in the proposal form . All companies give a copy of proposal form alongwith the policy document .
Please check filled proposal form and inform insurance company in writing in case ypu find any discrepancy.
