Is it possible to get a separate standalone health insurance policy for a 3 year old child?
Or any one of the parent also should be there in the plan?

Hope you are well! As Komal has suggested, it is not possible for a minor to get a standalone health insurance policy. They can get a coverage only with their parents.

In most cases, what Ayush and Komal have stated is correct.
However, I remember, there were a few options for insuring a child independently upwards of 5 yrs of age. Am not sure but please check Star Health and a couple of PSUs.

Insurance is basically a contract between you and the Insurer.
A minor cannot sign a contract, Right ?!
Hence, an adult + child, is the way to go
